Central Valley President's Scholarship Program - Required Documents

Family Income You can send a Copy of your SAR (Student Aid Report from FAFSA) as described above or send a copy of Parent's Income Tax return from this year or last year reflecting annual gross adjusted income and showing you as a dependant (Independent students allowed under FAFSA definition only). W2 Forms are not acceptable documents.

Official Transcript  Must come in a sealed envelope or have a school seal or official signature.  Transcripts must have student name and name of the institution. Transcripts MUST include your cumulative GPA on a 4.0 scale.  Please ask your school if your transcript includes a cumulative GPA on a 4.0 scale. If your transcript does not show a cumulative GPA, request that a school official confirm this information on school letterhead with his or her signature.
