Please review Eligibility & Deadlines to see which documents are required for each program and the postmark deadlines.
If you have multiple scholarship applications you only have to send one copy of each document.
Official Transcript Must come in a sealed envelope or have a school seal or official signature.Transcripts must have name, social security number, and name of the institution. Transcripts MUST include your cumulative GPA on a 4.0 scale.Please ask your school if your transcript includes a cumulative GPA on a 4.0 scale. If your transcript does not show a cumulative GPA, request that a school official confirm this information on school letterhead with his or her signature.
Proof of Citizenship If you applied for financial aid (FAFSA) you will receive a SAR. Please send a copy of ALL pages of this year's or last year's Student Aid Report (SAR). If you don’t have a SAR, you must provide some other proof of U.S. citizenship (copy of birth certificate, passport, naturalization certificate, certificate of Indian blood, or certificate of tribal affiliation) or U.S. permanent residency (copy of your alien identification card).You need to submit only one form of documentation. Please mail this document.
Family Income You can send a Copy of your SAR (Student Aid Report from FAFSA) as described above or send a copy of Parent's Income Tax return from this year or last year reflecting annual gross adjusted income and showing you as a dependant (Independent students allowed under FAFSA definition only). W2 Forms are not acceptable documents.
