Description of Documents
If you have multiple scholarship applications you only have to send one copy of each document.
Official Transcript
• Must come in a sealed envelope or have a school seal or official signature.
• Transcripts must have name, social security number, and name of the institution.
• Transcripts MUST include your cumulative GPA on a 4.0 scale. If your transcript does not show a cumulative GPA, request that a school official confirm this information on school letterhead with his or her signature.
Sample Form:
Proof of Household Income - Please submit one of the following:
• Student Aid Report (SAR) - If you applied for the Free Application for Federal Student Aid (FAFSA) you will receive a SAR. Please mail a copy of ALL pages of this year's or last year's SAR.
• Parent's Income Tax Return - Mail in your parent's income tax return from this year or last year reflecting annual gross adjusted income and showing you as a dependent.
(Independent students allowed under FAFSA definition only).
W2 Forms are NOT acceptable documents.

Proof of Citizenship - Please submit one of the following:
• Student Aid Report (SAR) from FAFSA
• Copy of birth certificate
• passport, or
• naturalization certificate
For proof of U.S. permanent residency submit a copy of your alien identification card.
